Recent repairs failed, 4in deep pothole on pavement

Recent repair work has failed, big hole has opened up. Elderly walk in this location frequently as it is on path from warden assisted housing. Hole is approximately 4 inches deep and 7-8 inch circumference. Been like this for a couple of days.
