Cover on footpath sticking out - and up - causing trip hazard

This cover was put in around a year or two ago I think, and they tarmacked around it, but the tarmac has degraded quite badly, and it's left the cover sticking up quite badly, and it is a trip hazard. Could be dangerous for anyone, but particularly the elderly, and disabled, and people with eyesight issues.
