Sunken pavement surface outside property boundary

Sent to Shropshire Council less than a minute later. Council ref: 14185453.

Pavement surface has worn down and in poor condition outside No.5 Gorse Lane in Bayston Hill, but more concerning is the sunken dip which has gradually happened over time of approx 4 inches at the lowest point against the top of our horizontal drive edge kerb stone. My partner uses a mobility scooter and cannot access this part of our front, also her carers have claimed its a trip hazard and we are liable for their welfare.
