Uneven footway surface. Slab face higher than should be due to damaged slabs

Sent to Shropshire Council less than a minute later. Council ref: 14134186.

These slabs are not in the best condition. This slab had an edge on it that meant I tripped and have a serious injury. Hospitalised following the trip on even surface. The slabs in question are on the footway outside of the library in the carpark travelling down hill towards Tesco. This issue in the footway appears to have been here some time with the build up of dirt. The slabs have not been recently damaged that is clear. Can someone call, make contact to discuss please as the injury sustained is severe.
