Unsafe condition. grid cover

I noticed this grid cover today 19-9-23. As you can see from the picture the cover has been recently lifted and not been replaced correctly. It is currently at least a trip hazard if not a potentially life threatening should the cover fall in if someone walks on it. Perhaps you would be good enough to identify the idle oik that left it in this condition and have him or her return to the task and replace it correctly.
